At 360 Therapy, we are committed to helping our patients regain their independence. Our therapists evaluate and treat patients on a one-on-one basis to create a specialized individual treatment plan. Our goal is to help each patient reach an optimal level of function.

Our Team

Melissa Tate, PT

Owner / Physical Therapist

A native of Tickfaw, Melissa obtained her bachelor’s degree in Kinesiology at Louisiana State University in Baton Rouge in 2001. She then went on to obtain her master’s degree in Physical Therapy from LSU Health Sciences Center of New Orleans in 2003.

 

Melissa has 16 years of experience serving patients of all ages, in the inpatient and outpatient settings, as well as within the home. In addition, Melissa has also spent the last 8 years as a pediatric provider for Early Steps, treating pediatric patients with varying diagnoses. Melissa resides in Loranger with her husband and children.

Richard Wale, PTA

Owner / Physical Therapist Assistant

A Ponchatoula native, Richard attended Our Lady of the Lake College in Baton Rouge, where he obtained an Associate of Science degree as a Physical Therapy Assistant in 2002. Richard has 17 years of experience treating patients in the inpatient, outpatient, and home health care settings. He resides in Hammond with his wife Sandy, stepson Cameron, and their son Jaxon.

David Hebert, PTA

Physical Therapist Assistant

A Houma native, David attended Delgado Community College in New Orleans, where he obtained an Associate of Applied Science as a Physical Therapist Assistant in 2007.  David has 12 years of experience treating patients in the inpatient, outpatient, and home health care settings.  He resides in Ponchatoula with his wife Jacki and their daughters, Olivia and Violet.

Eileen Willoughby, OT

Occupational Therapist

Eileen is a native of New York, New York. She attended New York University, where she obtained both her undergraduate (1983) and graduate (1985) degrees in Occupational Therapy. She has been a Licensed Occupational Therapist in Louisiana for the last 31 years. Her extensive experience includes serving patients of all ages, in the inpatient, outpatient, and home health care settings. She currently resides in Hammond with her husband, Mark.

Monica Brechtel, OT

Occupational Therapist

A Destrehan native, Monica attended LSU Health Sciences Center in New Orleans, where she obtained her bachelor's degree as an Occupational Therapist in 1995.  Monica has 24 years of experience treating patients inpatient, outpatient, and home care settings.  She resides in Ponchatoula with her two sons, Jacques and Jules.

Keely Puma, MS-CCC, SLP

Speech Therapist

A native of Baton Rouge, Keely obtained her bachelor's degree in Communication Disorders at LSU in Baton Rouge in 2003.  She then went on to obtain her master's degree in Speech-Language Pathology at LSU in 2005.

Keely has 14 years of experience of treating patients.  She resides in Tickfaw on a small farm with her husband.

Anthony Lamarca, PT, DPT

Physical Therapist

A native of Loranger, Anthony obtained his bachelor's degree in Kinesiology at Southeastern Louisiana University in Hammond in 2007.  He then went on to obtain his Doctorate of Physical Therapy from LSU Health Sciences Center of New Orleans in 2011.

 

Anthony has 8 years of experience serving patients in the inpatient and outpatient settings, as well as within the home.  Anthony resides in Loranger with his wife Alex and their daughters, Adelyn and Amalie.
