At 360 Therapy, we are committed to helping our patients regain their independence. Our therapists evaluate and treat patients on a one-on-one basis to create a specialized individual treatment plan. Our goal is to help each patient reach an optimal level of function.

Melissa Tate, PT

Owner / Physical Therapist

A native of Tickfaw, Melissa obtained her bachelor's degree in Kinesiology at Louisiana State University in Baton Rouge in 2001.  She then went on to obtain her master's degree in Physical Therapy from LSU Health Sciences Center of New Orleans in 2003.

Melissa has 21 years of experience serving patients of all ages, in the inpatient and outpatient settings, as well as within the home.  In addition, Melissa has also spent the last 12 years as a pediatric provider for Early Steps, treating pediatric patients with varying diagnoses.  Melissa resides in Loranger with her husband and children.

Anthony LaMarca, DPT

Physical Therapist

A native of Loranger, Anthony obtained his bachelor's degree in Kinesiology at Southeastern Louisiana University in Hammond in 2007. He then went on to obtain his Doctorate of Physical Therapy from LSU Health Sciences Center of New Orleans in 2011.
 
Anthony has 13 years of experience serving patients in the inpatient and outpatient settings, as well as within the home.  Anthony resides in Loranger with his wife Alex and their daughters, Adelyn and Amalie.

David Hebert, PTA

Physical Therapist Assistant

A Houma native, David attended Delgado Community College in New Orleans, where he obtained an Associate of Applied Science as a Physical Therapist Assistant in 2007. 
 
David has 17 years of experience treating patients in the inpatient, outpatient, and home health care settings.  He resides in Hammond with his wife Jacki and their daughters, Olivia and Violet.

Daniel Cookmeyer, PTA

Physical Therapist Assistant

A New Orleans native, Danny attended Delgado Community College, where he obtained an Associate of Applied Science as a Physical Therapist Assistant in 2006. 
 
Danny has 18 years of experience treating patients in the inpatient, outpatient, and home health care settings.  He resides in Ponchatoula with his wife Nichole and their children, Matthew and Emily.

Eileen Willoughby, OT

Occupational Therapist

Eileen is a native of New York, New York.  She attended New York University, where she obtained both her undergraduate (1983) and graduate (1985) degrees in Occupational Therapy. 
 
She has been a Licensed Occupational Therapist in Louisiana for the last 36 years.  Her extensive experience includes serving patients of all ages, in the inpatient, outpatient, and home health care settings.  She currently resides in Hammond with her husband, Mark.
